Please note that payment is due by November 10, 2019

Vendor Refund Policy:
Full refund for cancellations before October 4th, 2019.
50% refund for cancellations between October 5th and November 22nd, 2019.
No Refunds for cancellations after November 22nd, 2019 – no exceptions.

Please have a copy of your Seller’s Permit the day of the event: State Certification Form BOE 41* (follow the link below to get a temporary seller’s permit)
*Please note that certain sellers, such as Direct Sellers and Sales Agents, are exempt from holding a seller’s permit per the California State Board of Equalization.

We reserve the right to deny participation to any vendor for any reason.
By submitting this form, you, the vendor, release Prospect High School and the Campbell Union High School District from any liability for any loss or damage during the Prospect High School Holiday Craft Faire.
